Job Summary A hospital client based in Trenton, NJ has an onsite contract opportunity for a Master Social Worker . Shift schedule is Monday thru Friday 8:00 AM to 4:30 PM. This is a 13 week contract with possibility of extension. The Master Social Worker is an active part of the multidisciplinary team and is focused on discharge planning and coordinating post acute services for the patients. This position is for a PACE Center. Job Qualification Required

• Master's in Social Work Required
• Minimum of 1+ years experience in a hospital or healthcare setting as a Social Worker. Required
• Minimum of 1 year experience working with the frail or elderly population. Required
• Reliable Transportation as home visits are required for this role. Preferred
• Licensed Social Worker or Licensed Clinical Social Worker
