Full Job Description Social Worker/ Care Companion Job Summary:
Join Carease Health's Collaborative Care team as a Social Worker/Care Companion and make a meaningful impact on the lives of older adults in skilled nursing facilities. As a compassionate and dedicated Mental Health Provider, you'll provide emotional support, brief therapeutic interventions, and mental health assessments to help residents feel heard, supported, and emotionally connected. We're seeking a dedicated professional who shares our mission to elevate patient care standards and improve overall well-being.
Key Responsibilities:
Visit designated facilities in accordance with the schedule set with your supervisor Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns Document visit notes and maintain detailed records of interactions and interventions after each visit Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric
NP Qualifications:
Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings Strong communication, documentation, and relationship-building skills Why Join Carease Health?
