Job Description

Job Summary:
We are seeking a compassionate and dedicated Social Worker to join our team at The center for Rehabilitation and nursing and Washington township. The Social Worker will play a crucial role in supporting the emotional, social, and psychological well-being of our residents and their families. This individual will ensure that residents receive the necessary services and resources to maintain their quality of life while adhering to all federal, state, and local regulations.
Key Responsibilities:
Conduct psychosocial assessments to identify residents' needs and develop individualized care plans. Provide emotional support, counseling, and crisis intervention for residents and families. Advocate for residents' rights and ensure their needs and preferences are met. Coordinate discharge planning and transitions to other levels of care as needed. Collaborate with medical staff, therapists, and other professionals to provide comprehensive care. Assist residents and families in navigating Medicaid, Medicare, and other financial assistance programs. Organize and lead support groups or educational sessions for residents and families. Maintain accurate and confidential records in compliance with regulations and facility policies. Address and resolve resident grievances in a timely and effective manner. Participate in care planning meetings and interdisciplinary team discussions.
Qualifications:
Bachelor's or Master's degree in Social Work (BSW/MSW) from an accredited institution. Valid state licensure or certification as required. Experience in a nursing home, long-term care, or healthcare setting preferred. Strong knowledge of state and federal regulations related to social work in healthcare. Excellent communication, interpersonal, and problem-solving skills. Ability to work independently and as part of a multidisciplinary team. Proficiency in electronic medical records (EMR) and documentation.
