About the Role At Corus Health, we walk alongside families through some of the most profound moments of human experience. As a Bereavement Coordinator, you'll be the steady, compassionate presence that helps families find their footing after loss — through individual counseling, group support, community outreach, and the kind of care that lingers long after a patient's passing. This role sits at the heart of what hospice is really about, and it calls for someone who brings both clinical grounding and genuine human warmth to grief work every single day. Key Responsibilities Provide individual and group grief counseling to patients, families, and caregivers Assess bereavement needs and develop individualized support plans for each family Plan and facilitate support groups, memorial events, and educational workshops Recruit, train, and supervise volunteers supporting bereavement services Participate in Interdisciplinary Group (IDG) meetings to address bereavement needs across the care team Collaborate with hospice staff to integrate bereavement care into patient and family plans Deliver community outreach programs and represent Corus hospice bereavement services externally Maintain accurate, timely documentation in accordance with hospice policies and HIPAA regulations
Requirements:
What We're Looking For Bachelor's degree in Social Work, Counseling, Psychology, or a related field preferred Previous hospice bereavement experience or background as a chaplain (required) Minimum of 2 years in a related field, with experience or education in grief or loss counseling preferred Certification in grief counseling or bereavement care (e.g., thanatology) is a plus Experience facilitating group counseling or grief support sessions Strong knowledge of grief models, interventions, and best practices Cultural sensitivity and the ability to support families from diverse backgrounds Proficiency with EMR systems and Microsoft Office; valid driver's license and reliable transportation Someone who can hold space for others in their hardest moments — with steadiness, empathy, and care Why Join Us Corus Health offers a rare combination in home-based care: home health, hospice, and primary care under one roof in Albuquerque.
