Job Description

Overview We are seeking a compassionate and dedicated Social Worker to join our team. The ideal candidate will have a strong background in Dialysis and a passion for helping individuals and families navigate their challenges. This role involves providing therapeutic support, conducting assessments, and developing treatment plans tailored to the needs of clients. The Social Worker will work collaboratively with other healthcare professionals to ensure comprehensive care and support. Duties Perform patient assessments to identify needs and develop appropriate treatment plans. Provide crisis intervention services to clients in need of immediate support. Engage in motivational interviewing to encourage positive behavioral changes among clients. Collaborate with multidisciplinary teams to coordinate care for patients, including those with developmental disabilities, PTSD, and addiction issues. Maintain accurate medical documentation in compliance with HIPAA regulations, including intake forms, progress notes, and treatment plans. Utilize EMR systems for data collection and management of medical records. Participate in clinical research initiatives aimed at improving patient outcomes. Offer home and community care services to support clients in their natural environments. Provide education and resources to clients regarding mental health issues, coping strategies, and available community resources. Qualifications Master's degree in Social Work (MSW) from an accredited program. Valid state licensure as a clinical social worker (LCSW) preferred. Knowledge of medical terminology, CPT coding, ICD-9/ICD-10 coding, and psychopharmacology is advantageous. Strong skills in case management, diagnostic evaluation, crisis management, and patient care. Ability to conduct clinical research and collect relevant data for program improvement. Excellent communication skills, both verbal and written, along with strong interpersonal skills to build rapport with clients.
