Job Duties/Responsibilities:
Demonstrates ability, and is responsible to perform psychosocial assessments on Medicare, Medicaid and any high-risk patients to determine social service and possible discharge planning needs. Documentation of assessments and interventions is timely and meets current practice standards and policies. Maintains current knowledge of resources available within the community; maintains supply of resource materials to be distributed to patients when needed. Is able to obtain other resources as needed. Communicates with family members and caretakers regarding the needs of the patient and anticipated plans. Involves patient and family in the plan of care. Graduate from an accredited school of nursing, associate's degree required, baccalaureate degree preferred. Current Nevada Registered Nurse License. Five (5) or more years of previous experience in all aspects of nursing and two (2) or more years of UR or CM experience preferred Current BLS certificationOR Social Worker:
Masters Social Work (MSW) preferred, BSW required. Current state Social Worker licensure. 3 or more years of previous experience in all aspects of social service and discharge planning preferably in an acute care setting.
