assignment . $40 per hour based on skills and experience.

Social Worker Qualifications:

Must have a Bachelor's Degree in Social Work or related field. One year of Social Work in Long-Term Care required. Experience using PCC is a plus.

Social Worker Introduction:

Under the supervision of the Director of Social Work, assist the residents to attain or maintain the highest practicable physical, mental and psychosocial well-being, and in navigating the health care system.

Social Worker Job Summary:

Interview residents and their families. Provide assessments and counseling to residents and families. Develop care plans based on the individual need of residents. Provide crisis intervention as needed. Provide grief and supportive counseling. Coordination of discharge planning and case management.
