$70,000-$75,000 annually Join Our Team as a Social Worker Briarcliff Manor Center for Rehabilitation and Nursing Care is seeking a compassionate and dedicated Social Worker to join our team. In this role, you will play a vital part in supporting the emotional, psychological, and social well-being of our residents while collaborating closely with families and the interdisciplinary care team. Key Responsibilities Conduct interviews with residents and families to gather social history and assess needs Engage residents and families in social service planning whenever possible Provide guidance and solutions for social and environmental challenges, including financial assistance, discharge planning, and referrals to community agencies Collaborate with administration, medical, nursing, and other departments to coordinate comprehensive care plans Communicate residents' social, psychological, and emotional needs to physicians and the interdisciplinary care team Participate in facility surveys and inspections conducted by authorized government agencies Assist in the development and implementation of social service programs and activities Qualifications Master's degree in Social Work preferred At minimum, a Bachelor's degree in Social Work, Sociology, Special Education, Rehabilitation Counseling, Psychology, or a related field Minimum of one (1) year of supervised social work experience in a healthcare setting, working directly with individuals Prior nursing home experience required Membership in good standing with the National Association of Social Workers (NASW) and the Academy of Certified Social Workers preferred Benefits Comprehensive Medical & Dental Coverage Generous Paid Time Off (PTO) 401(k) Retirement Plan Employee Recognition Program Supportive, team-oriented work environment Company perks and employee discounts If you are passionate about advocating for residents and making a meaningful difference in long-term care, we encourage you to apply today!
