Job Description

Schedule:
Monday through Friday 9:30AM - 6:00PM 30-minute unpaid break Late clinic hours Wednesday and Thursday - must work one late night per week, can either work adjusted hours or take overtime

ClinicalWorks Responsibilities:
Ensure appropriate services are delivered to the program's patients Provide individual health education sessions to the patients within the program Conduct assessments, reassessments, conferences, and reasoning for participation in health education sessions Develop and update patients' individual service and care plans based on assessments Lead and be responsible for quality-of-service delivery to patients, required documentation, and recruitment goals Work closely with the Program Manager for Support Services to ensure appropriate services are delivered to patients within the program (this includes conducting program orientation, screening/rescreening, intakes, and reasoning for participating in program services) Provide psychoeducation support groups to the patients within the program Assist the Program Manager for Support Services to coordinate and attend all required training sessions and meetings (such as care planning, ad hoc, training, and supervision) Work closely with organization staff and other service providers to coordinate patient care Manage patient satisfaction and exit surveys, complete reports, and prepare the results Manage and monitor the program's key deliverables and meet the program's benchmarks Maintain accurate records on services and activities in designated database and reporting structures (attendance, registration, patient interactions, and other required documentation) Write and submit weekly reports adhering to deadlines Work as a part of the team for quality assurance and continue quality improvement tasks Communicate effectively with patients, families, and the public to build trusting relationships across a broad range of socioeconomic and cultural backgrounds Represent the organization at various venues and promote agency services Other activities as assigned

$28.00 - $34.00 per hour
