A Social Worker is responsible to act as a liaison for the residents social interests and to plan, organize, and attend to the psychosocial needs of the residents in accordance with current federal, state and local standards. Responsibilities:
Responsible for maintaining documentation of social work activities as required by state and federal rules and regulations.

Participates in ongoing evaluations and quality assurance projects.

May assist with the development of departmental policies and procedures to ensure the highest quality of care to the residentsHelps determine any unmet psychosocial needs of the residents through regular contact with residents and/or the family and through staff referrals. Arranges help as needed through internal and external sources.

Qualifications:
Bachelor degree in a related field or minimum 3 years experience in the social work field.

Education or experience in long term care preferred. Ability to lift 60 pounds if necessary with the ability to sit, walk, bend, and twist.
