Role Highlights Located at our modern Mineola clinic A dynamic patient mix: psychosocial assessments + crisis intervention AI-powered documentation tools to reduce charting time A team culture with strong leadership presence Compensation + Benefits $65K-$85K base salary (depending on experience and license level) Medical, dental, vision, life insurance + 401(k) PTO + paid leave programs Free & discounted care for you and your family What You'll Do Evaluate, assess, and support acute + chronic social determinants of health for the Mineola community Connect patients to local community resources, housing, and financial assistance Create care coordination plans and coordinate follow-up care Educate patients and families on mental wellness and systemic healthcare navigation Document visits in EMR using technology-driven tools Master's Degree in Social Work (MSW) from an accredited institution Active New York State license (LMSW or LCSW) Board certification or state-approved licensure in good standing EMR experience and strong HIPAA knowledge Excellent communication, active listening, and crisis-de-escalation skills

Preferred:
