(WSFSSH) seeks a Social Worker II to provide person-centered case management for older adults and other residents in our supportive and affordable housing programs. Reporting to the Community & Social Services department, this role conducts biopsychosocial assessments, develops care plans, coordinates benefits and community resources, leads groups, and advocates for residents' rights and well-being. The Social Worker II collaborates closely with on-site teams, maintains accurate documentation, and contributes to a supportive, inclusive culture where diversity is valued and all employees are empowered.
