Navitas Healthcare, LLC is seeking Licensed Master Social Worker (LMSW) for an exciting Travel or Local job in New York, NY. Responsibilities Conduct comprehensive psychosocial assessments to evaluate patients' emotional, social, and environmental needs upon admission Provide supportive counseling, crisis intervention, and emotional support to patients and families Coordinate safe and effective discharge planning for patients across inpatient units Facilitate referrals to community resources, home care services, long-term care facilities, and outpatient programs Collaborate with physicians, nurses, therapists, and interdisciplinary healthcare teams to support patient care goals Maintain accurate and timely documentation of assessments, interventions, and discharge plans Monitor patient progress and address barriers impacting discharge and continuity of care Advocate for patients and families to ensure access to appropriate services and resources Participate in care conferences and interdisciplinary meetings as required Requirements Master's Degree in Social Work (MSW) from an accredited institution required Current Licensed Master Social Worker (LMSW) License issued by the New York State Education Department (NYSED) required Minimum 1 year of LMSW experience required Strong psychosocial assessment, discharge planning, and case management skills Experience coordinating referrals and community resource services Excellent communication, interpersonal, and documentation skills Ability to work effectively in a multidisciplinary healthcare environment English proficiency required (read, write, and speak) Preferred Qualifications Acute care hospital experience preferred Experience managing inpatient Medicine, Surgery (Med/Surg), or Rehabilitation caseloads preferred Strong knowledge of discharge planning, care coordination, and community resource networks preferred For more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.
