PM Hours:
35 Hours per Week Job Summary "Navitas Healthcare, LLC" is seeking an experienced Licensed Master Social Worker (LMSW) to support patients within an inpatient medicine/surgery setting. The LMSW will provide psychosocial assessments, supportive counseling, discharge planning, and referral coordination to ensure patients receive comprehensive care and support throughout their hospitalization. Key Responsibilities Conduct psychosocial assessments for inpatient medical and surgical patients Provide supportive counseling and therapeutic interventions Develop and coordinate discharge plans Make referrals to community resources and support services Collaborate with physicians, nurses, case managers, and interdisciplinary teams Document assessments, interventions, and patient progress accurately Advocate for patients and assist with care coordination needs Support continuity of care and transition planning Required Qualifications Master's Degree in Social Work (MSW) Active New York State Licensed Master Social Worker (LMSW) license Minimum 1 year of LMSW experience Strong communication, counseling, and documentation skills Ability to work effectively in an inpatient hospital environment For more details reach at sthakur@navitashealth.com
