Job Description

Salary Range:
$62,360-$65,000 (Pay information will vary depending upon relevant experience for the position) Full Time 40 hours Ranked #1 Nursing Home in NYS by Newsweek's Best Nursing Homes CMS 5 star ranking 401(k), tuition reimbursement, additional employee perks and benefits
GENERAL SCOPE
General Responsibilities include case management and therapeutic intervention in the psychosocial environment of patients on short-term rehab to promote the patients optimal physical, psychological and social functioning throughout their rehabilitation.
EDUCATION / QUALIFICATION
Bachelor's Degree in Social Work required. Master's Degree preferred.
EXPERIENCE
Prefer one (1) year experience in a long-term care setting or related setting.
