Job Description

Case Coordinator Rehabilitation Support Services, Inc. is hiring a Case Coordinator for its Apartment Program. Hours and Benefits:
  • 40 hour work week, Monday through Friday

Case Coordinator:
  • A high school diploma or equivalent
  • Valid NYS Driver's License required for most positions.
  • Successful clearance by the Justice Center and New York State Office of Children and Family Services Statewide Central Register Database Check also required. Responsibilities for the Case Coordinator include :
  • Responsible for a designated case load; including the accurate and timely completion of case record documentation in the Agency electronic health record.
  • Establish and maintain good rapport and therapeutic relationship with residents.
  • Aid and encourage clients to meet the established goals of their service plans.
  • Teach, monitor and evaluate client's skills of daily living.
