Skilled Nursing Facility Salary:

Up to $70,000 per year The Willows at Ramapo Rehabilitation and Nursing Center is seeking a compassionate and motivated Social Worker to join our interdisciplinary team. Position Overview The Social Worker is responsible for addressing residents' psychosocial needs, assisting with discharge planning, and collaborating with the care team to ensure the highest level of resident support and satisfaction. Key Responsibilities Assess residents' social, emotional, and psychosocial needs Develop and maintain individualized care plans Participate in interdisciplinary care plan meetings Coordinate discharge planning and transitions of care Assist residents and families with community resources, home care services, and discharge arrangements Provide emotional support and counseling to residents and families Advocate for resident rights and overall well-being Maintain accurate documentation in compliance with facility policies and regulations Collaborate with nursing, therapy, and administrative teams to support quality resident care Qualifications Bachelor's Degree in Social Work (BSW) or related field required; MSW preferred Prior experience in a skilled nursing facility, rehabilitation center, or long-term care setting preferred Strong communication and interpersonal skills Knowledge of discharge planning and care coordination Ability to multitask and work effectively in a fast-paced environment Experience with PCC and MDS 3.0 is a plus Compensation & Benefits Salary up to $70,000 annually (based on experience) Competitive benefits package including: Medical, Dental, and Vision Insurance Paid Time Off 401(k) Retirement Plan Employee Perks and Discount Programs Opportunities for professional growth and advancement The Willows at Ramapo Rehabilitation and Nursing Center is proud to be an Equal Opportunity Employer.
