Location would be in Utica, 1229 Kemble St. traveling around assigned portfolio. This is a Full Time benefit eligible position, Monday

• Friday 8:30 am
• 5:00pm, with occasional night/weekend hours. Pay Range is $25

JOB SUMMARY

The Housing Stability Coordinator supports individuals experiencing housing instability, with a primary focus on Housing Visions' low-moderate income households and Medicaid members. This role involves conducting trauma-informed assessments and developing individualized service plans to increase self-sufficiency and promote independent living, along with screening for health-related social care needs through the

NYS 1115

Medicaid Waiver. The Housing Stability Coordinator provides ongoing case management, advocacy, safety planning, and crisis intervention, as well as community-building activities and connections to community-based resources. The overall goal of CONNECT is to support residents in securing and maintaining safe, quality, affordable housing and fostering a supportive environment which reflects the mission, vision, and values of Housing Visions.
