Job Description

Hutchings Psychiatric Center (HPC), an Office of Mental Health (OMH) Joint Commission accredited facility, is recruiting a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) to join their Mobile Integration Team (MIT). The MIT program at HPC offers a network of community-based services that support patient-centered treatment. Licensed Social Workers are vital members of the multidisciplinary MIT team whose focus is providing clinical and support services to individuals diagnosed with mental illness to help them successfully reach their treatment goals. Duties include, but are not limited to: Providing therapeutic support using evidence-based practices. Teaching symptom management, coping, and medication management skills. Developing and implementing community groups. Coordinating services with providers and linking individuals to community-based services. Working collaboratively with clinic staff to perform wellness checks. Documenting daily contact notes, service recordings, and correspondences. Completing individual service plans, progress notes, and referral forms. Minimum Qualifications:
Licensed Master Social Worker- Licensed and currently registered as a Licensed Master Social Worker in New York. Licensed Clinical Social Worker- Licensed and currently registered as a Licensed Clinical Social Worker in New York. Pay:
$74,193.00 - $109,650.00 per year
