Travel Licensed Master Social Worker (LMSW) Valhalla, NY | Start ASAP | 30-Week Assignment HumanEdge Health is seeking a qualified Travel LMSW for a long-term assignment supporting the Clinical Care Management team in Valhalla, NY. This is a full-time, weekday position offering consistent hours in a clinical care environment.

Clinical Care Management Role Overview Provide clinical social work services within a care management setting Coordinate care planning and support patient discharge and transitions of care Collaborate with interdisciplinary healthcare teams Assess psychosocial needs and connect patients with appropriate resources Maintain accurate documentation in accordance with regulatory standards Requirements Master's degree in Social Work (MSW) from a CSWE-accredited program Active New York State LMSW or LCSW license and current registration required Strong clinical assessment, communication, and documentation skills Healthcare or care management experience preferred
