Based on the state mandated salary schedule according to degree level and years of teaching experience ($40,838.00
  • $71,037.
00 per year) plus competitive benefits package equal to approximately 35% of direct compensation. The Title I Transition Counselor provides comprehensive support to students and families through key educational transitions from early childhood through post-secondary planning while also serving as a critical support in improving student attendance and engagement. This position strengthens academic success, social-emotional well-being, attendance, and family-school partnerships by offering targeted counseling, structured transition programming, attendance intervention, and inclusive family engagement opportunities. The counselor serves as a bridge between home, school, and community resources to ensure continuity of support for all students, with an emphasis on Title I populations. This position works collaboratively with school administrators, the Attendance Director, and the Federal Programs Director to ensure compliance with state attendance requirements, Title I regulations, and county strategic plan goals. Job Duties Student Transition Support Design and implement transition programs for: Home to early childhood programs Plans and implements Tot Spot program in collaboration with community partners Elementary to Middle School Middle School to High School High School to Postsecondary (college, career, military) Provide transition support for students entering, enrolled in, and exiting alternative education programs, including: Regular check-ins alternative education students Development of individualized transition plans to support reentry into the regular education environment Coordination with school counselors, administrators, and teachers to ensure academic, behavioral, and attendance readiness for transition Participate in MTSS meetings of support students with academic, attendance, behavioral, or social-emotional needs Works with school counselors for coordinating school behavior plans and school-home plans Provide individual and small-group counseling for students and families focused on: Adjustment, anxiety, and emotional regulation Academic readiness and organizational skills Goal setting, attendance habits, and self-advocacy Identify students at risk during transitions due to attendance, behavior, or academic concerns and develop targeted support plans. Collaborate with counselors, teachers, administrators, and support staff to ensure continuity of services and reduce barriers impacting attendance and engagement. Meet with students returning from juvenile placements, residential programs, or mental health facilities to support successful reintegration into the school environment Coordinate reentry planning meetings with families, school staff, counselors, administrators, and outside agencies as appropriate Support students in developing reentry transition plans addressing: Attendance expectations and compliance Social-emotional and behavioral supports School expectations Serve as a consistent point of contact for families during the reentry process to promote stability, engagement, and continuity of services Monitor student progress following reentry and adjust supports as needed to promote successful long-term placement Serve as the county contact for Multi-Disciplinary Team meetings (MDT) Collaborative Student Support & Family Engagement The Title I Transition Counselor and the County-Wide Elementary Communities in Schools/Student Support Liaison work collaboratively to strengthen family engagement, remove barriers to student success, and coordinate county-wide student support efforts.
Responsibilities include:
Collaborate in planning and implementing county-wide family engagement events that support student attendance, social-emotional wellness, and academic success. Coordinate outreach efforts to ensure families with identified needs receive consistent communication and support across schools and programs. Share data and information regarding attendance concerns, student transitions, and family support needs to ensure coordinated intervention strategies. Conduct joint home visits when appropriate to address significant barriers impacting attendance, engagement, or student well-being. Coordinate with school administrators, counselors, and community agencies to ensure aligned support services for students and families. Collaborate to identify systemic barriers affecting students and families and develop strategies to improve school-family partnerships. Participate in regular collaboration meetings to review student needs, attendance trends, and family engagement opportunities. Work collaboratively with the CIS Coordinator to ensure elementary family engagement efforts align with student support needs identified through the CIS model and SAT processes. Attendance Support Support county and school-level attendance improvement initiatives aligned with state law, Title I requirements, and strategic plan goals. Monitor and respond to attendance concerns for identified students, particularly those experiencing chronic absenteeism. Coordinate closely with the Attendance Director and Federal Programs Director to: Align attendance interventions with federally funded supports Support accurate documentation and reporting Conduct home visits as appropriate to address attendance barriers, strengthen family engagement, and connect families with needed resources. Collaborate with school administrators and support teams to develop attendance improvement plans for students and families. Serve as a family-facing support to reduce truancy, improve daily attendance, and promote consistent school engagement. Family Engagement & SEL Programming Lead the planning and coordination of county-wide family engagement events in collaboration with the Communities in Schools/Student Support Liaison school teams. Ensure events are intentionally aligned with Social Emotional Learning (SEL ) and focus on: Educating families on ways to support their child's success Strengthening family-school relationships Improving communication between families and schools Supporting emotional regulation, healthy relationships, and student well-being Promoting the importance of regular school attendance Collaborate with school staff to design developmentally appropriate events for elementary, middle, and high school families Develop and distribute family resources that support academic success, attendance, and positive behavior Conduct family outreach to address attendance concerns, academic barriers, and engagement needs; assist families in navigating school systems, attendance expectations, and available supports Assist families with scheduling and participating in meetings, conferences, and due-process related discussions as needed Maintain documentation of student and family contacts related to attendance, behavior, and academic interventions Assist school administrators with planning family engagement activities aligned to school improvement goals and in compliance with Title I regulations Title I & Community Collaboration Attend and participate in advisory council meetings as appropriate Build and maintain strong partnerships with community agencies and service providers, including but not limited to: Head Start Family Resource Networks Family Support Center/Starting Points Healthy Grandfamilies Haven Youth Coalition Follow up with community partners to ensure continuity of services and collaborative supports for families Align services with Title I goals, funding requirements, and attendance improvement initiatives Connect families with school-based and community resources to address barriers affecting attendance, behavior, and academic success Maintain documentation and data related to family engagement, attendance interventions, participation, and outcomes Program Development & Reporting Work with appropriate school staff to develop annual transition, attendance support, and family engagement plans Maintain accurate and up-to-date documentation of parent contacts, student support services, family engagement events, and community collaborations Prepare and submit required weekly, monthly, and annual reports to the Federal Program Director and/or Attendance Director Assist with data collection, reporting, and evaluation of the Title I programs, as they relate to student supports, transitions, and community and family engagement initiatives Support compliance with all federal, state, and county Title I requirements, including audit and monitoring readiness Plan and conduct Annual Title I meetings and parent advisory meetings in collaboration with the Federal Program Director Assist with the development and review of the LEA Parent & Family Engagement Policy Assist with the development of the LEA Needs Assessment and Strategic Plan Other duties as assigned by the Superintendent Qualifications WVDE Professional Student Support Certificate endorsed in School Counselor PK-AD Experience working with children and families across multiple age levels Knowledge of Title I programs, attendance law, and family engagement best practices Strong communication, organization, documentation, and collaboration skills Work Environment School-based as assigned with county-wide collaboration (Central Office is base location) Evening events required for family engagement activities (schedule adjustments permitted with prior approval from Federal Programs Director) Travel between schools and home visits required (mileage reimbursed from base location) Coordination The Title I Transition Counselor and the County-Wide Elementary Communities in Schools/Student Support Liaison will maintain regular collaboration to ensure alignment of student support services, family engagement initiatives/events, and attendance improvement strategies across Webster County Schools.
