5x7s Job Summary "Navitas Healthcare, LLC" is seeking an experienced Licensed Master Social Worker to provide psychosocial assessments, supportive therapy, and discharge planning services in an inpatient medicine/surgery setting. Role focuses on patient support, care coordination, and referral management. Key Responsibilities Conduct psychosocial assessments for inpatient patients Provide supportive counseling and therapy Perform discharge planning and care coordination Make appropriate patient referrals to community resources Collaborate with interdisciplinary healthcare teams Document assessments and interventions in EMR Support inpatient medicine/surgery unit operations Ensure compliance with hospital social work standards Required Qualifications Master's Degree in Social Work (MSW) Active LMSW license (NYSED) Minimum 1 year LMSW experience Strong communication and documentation skills Experience in inpatient hospital setting preferred For more details reach at
