35 Hours Per Week Position Overview "Navitas Healthcare, LLC" is seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to support an inpatient medicine and surgery team in New York, NY. This role focuses on psychosocial assessments, discharge planning, patient advocacy, and connecting patients and families with appropriate community resources to ensure positive healthcare outcomes. Key Responsibilities Conduct comprehensive psychosocial assessments for patients and families. Provide supportive counseling and therapeutic interventions. Develop and implement individualized discharge plans. Coordinate referrals to community agencies, rehabilitation services, home care providers, and support programs. Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams. Advocate for patients and families to ensure access to appropriate resources and services. Assist patients and families in coping with illness, hospitalization, and recovery. Maintain accurate, timely, and confidential documentation. Participate in multidisciplinary rounds, care conferences, and treatment planning meetings. Monitor patient progress and adjust interventions as needed. Ensure compliance with professional social work standards, ethics, and hospital policies. Required Qualifications Master's Degree in Social Work (MSW) from an accredited institution. Active Licensed Master Social Worker (LMSW) license issued by the New York State Education Department (NYSED). Primary Source Verification of LMSW License. Minimum 1 year of LMSW experience . Strong knowledge of psychosocial assessments, counseling techniques, and discharge planning. Excellent communication, interpersonal, and organizational skills. Ability to work effectively within a multidisciplinary healthcare environment. Experience with electronic medical records and clinical documentation.
