  • 60/hour Position Overview We are seeking an experienced Licensed Master Social Worker (LMSW) to provide comprehensive psychosocial services within an acute care hospital setting.
The ideal candidate will perform patient assessments, provide supportive counseling, coordinate discharge planning, and facilitate access to community resources while collaborating closely with interdisciplinary healthcare teams to support optimal patient outcomes. Responsibilities Conduct comprehensive psychosocial assessments for patients and their families. Evaluate emotional, social, environmental, and financial factors impacting patient care and recovery. Provide supportive counseling, crisis intervention, and emotional support services. Develop and coordinate discharge plans to ensure continuity of care. Facilitate referrals to community agencies, rehabilitation programs, support services, and other appropriate resources. Collaborate with physicians, nurses, case managers, therapists, and other healthcare professionals to develop patient-centered care plans. Advocate for patients and families while addressing barriers to care and treatment. Assist patients with adjustment to illness, hospitalization, and transition planning. Maintain timely, accurate, and thorough clinical documentation in accordance with regulatory and organizational standards. Participate in interdisciplinary team meetings and care conferences. Ensure compliance with social work ethics, professional standards, and healthcare regulations. Required Qualifications Education Master's Degree in Social Work (MSW) required. Licensure & Certifications Active New York State Licensed Master Social Worker (LMSW) license issued by the New York State Education Department (NYSED) required. Experience Minimum one (1) year of Licensed Master Social Worker experience required. Prior hospital experience required. Experience working in inpatient medicine, medical-surgical (Med/Surg), or rehabilitation settings required. Experience with discharge planning, care coordination, and referral management required. Skills & Competencies Strong psychosocial assessment and clinical interviewing skills. Experience providing supportive counseling and crisis intervention. Knowledge of discharge planning processes and community resource coordination. Strong understanding of healthcare systems and interdisciplinary care models. Excellent communication, interpersonal, and relationship-building skills. Strong documentation and case management abilities. Ability to work effectively with diverse patient populations and families. Strong organizational, problem-solving, and critical-thinking skills. Ability to manage multiple priorities in a fast-paced healthcare environment. Commitment to patient advocacy, ethical practice, and high-quality care.
