Social Worker Sans Souci Rehabilitation and Nursing Center is seeking a talented Social Worker to join our team

Social Worker Responsibilities Include:

Assumes authority, responsibility, and accountability for the delivery of medical related social services in the facility. Collaborates and coordinates in an efficient and economic sound process with the interdisciplinary team Identifying individual social and emotional needs Developing and maintaining individualized care plans Participating in the planning of the resident's admission, community re-entry or transfer to another facility Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident Work with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stay Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community

Social Worker Qualifications/Requirements:

BSW or MSW preferred, or Bachelors degree in related field At least one year experience in Social Work field required Long term care and/or sub-acute setting preferred Ability to work flexible schedule PCC experience preferred Knowledge of MDS 3.0 preferred
