Non-Licensed Social Worker

Franklin Plaza is a resident centered skilled nursing and long term care community committed to providing compassionate, high quality care. We are seeking a caring and organized Social Worker Designee to support residents and families with psychosocial needs, admissions, discharges, and care planning. The Social Worker Designee assists in managing the day to day social services operations. This role supports residents and families through transitions, care planning, problem-solving, and emotional support. The ideal candidate is compassionate, detail oriented, and comfortable working with seniors and their families. Key Responsibilities Assist with the psychosocial assessment of new residents Support the admission process, ensuring residents and families feel welcomed and informed Participate in care plan meetings and document resident needs and preferences Provide emotional support, crisis intervention, and problem-solving assistance Communicate with residents, families, and interdisciplinary team members Assist with discharge planning and coordination of services Maintain accurate documentation in accordance with state and federal regulations Advocate for residents' rights and help ensure a positive care experience Assist residents with completing forms, applications, and service coordination Participate in resident and family education regarding available resources Qualifications Experience in long-term care, social services, case management, or senior care preferred Ability to work with diverse resident populations and their families Strong communication, empathy, and interpersonal skills Organized, dependable, and able to maintain confidentiality Knowledge of state and federal long-term care regulations a plus Ability to work collaboratively with an interdisciplinary team What We Offer Competitive compensation Medical, dental, and vision insurance options Paid time off and holidays Retirement plan options Supportive and team-focused work environment Franklin Plaza - West Side of Cleveland Rooted in the
