Job Description

About Community Partnership on Aging Community Partnership on Aging (CPA) is a collaborative organization serving older adults in eight Northeast Ohio communities: Gates Mills, Highland Heights, Lyndhurst, Mayfield Heights, Mayfield Village, Richmond Heights, South Euclid, and University Heights. CPA provides a wide range of services designed to help older adults remain healthy, independent, and engaged, including transportation, social work services, congregate meals, homemaking assistance, health and wellness programs, food distribution, educational opportunities, and safe-at-home modifications. CPA is seeking a compassionate and dedicated Part-Time Social Worker to provide outreach, supportive services, information and referrals, and case management services to older adults and their families.
Schedule:
30 hours per week (flexible schedule)

OPERS, Workers' Compensation, and Medicare benefits Essential Responsibilities Conduct client assessments and develop individualized care plans as appropriate. Reassess client needs and update care plans in accordance with program requirements and client circumstances. Provide supportive services, advocacy, outreach, information, referrals, and case management. Coordinate referrals to community agencies and service providers. Provide supportive counseling to individuals and families; refer clients for therapeutic counseling when appropriate. Maintain accurate and timely case documentation, including client contacts and coordination with family members and service providers. Build and maintain positive relationships with clients, community partners, volunteers, and staff. Stay informed about resources and services available within CPA member communities. Participate in special programs and events, including occasional evening or off-hour activities with advance notice. Assist with student field education and training opportunities. Attend continuing education and professional development programs relevant to the position. Qualifications Current Ohio LSW or LISW license required. Experience working with older adults preferred. Ability to work independently, solve problems effectively, and respond appropriately in emergency situations. Strong interpersonal skills and demonstrated sensitivity to the needs of older adults. Proficiency with Microsoft Office Suite (Word, Excel, Outlook). Ability to utilize agency software for documentation, reporting, and case management. Pre-Employment Requirements Candidates receiving a conditional offer of employment must successfully complete a background check, fingerprint screening, and drug screen. CPA covers the cost of all required pre-employment screenings.
Pay:
$22.00 - $28.00 per hour
