Located in Cleveland's University Circle neighborhood, our 25-apartment campus serves individuals and families who travel to Northeast Ohio for transplant-related evaluation, treatment, recovery, and follow-up care. We are not a medical facility; we play an essential role in supporting patients and caregivers throughout the transplant journey by providing a safe place to stay, meaningful connections, and access to resources that promote well-being. Position Summary The Social Worker plays a vital role in supporting transplant patients and caregivers during one of the most challenging periods of their lives. This position provides emotional support, resource navigation, wellness education, advocacy, and community-building opportunities through individual interactions, support groups, and programming. As the sole social worker on campus, this individual serves as a trusted resource for guests while collaborating closely with staff, volunteers, hospital transplant teams, and community partners. Particular emphasis is placed on supporting caregivers, fostering resilience, encouraging self-efficacy, and helping guests access resources that improve emotional, social, and financial well-being. This position offers significant autonomy, creativity, and the opportunity to make a meaningful impact on the lives of transplant patients and their families. Essential Responsibilities Guest Support & Resource Navigation Build supportive relationships with transplant caregivers and patients throughout their stay. Proactively conduct ongoing needs assessments and connect guests with appropriate resources and services. Provide one-on-one support focused on overall wellness and quality of life. Offer guidance related to insurance, financial assistance, fundraising resources, and grant opportunities. Administer the organization's Family Assistance Program. Provide crisis intervention and support during challenging situations. Encourage self-advocacy and connect guests with educational resources, support groups, and professional services when appropriate. Community Building & Programming Foster opportunities for meaningful guest engagement, connection and a welcoming, inclusive community. Facilitate support groups and peer-to-peer connections. Coordinate and support weekly Alcoholics Anonymous meetings. Lead the weekly caregiver support group, Navigating the Caregiver Journey . Collaborate with staff, volunteers, the Programming Committee, and community partners to develop educational and social programming. Attend guest meals, social activities, and events to promote connection and engagement. Volunteer & Staff Engagement Recruit, vet, train, supervise, and support volunteers involved in programming and guest services. Provide guidance and mentorship to guest-facing staff and volunteers. Lead the Programming Committee and help shape the organization's programming strategy. Promote a trauma-informed, guest-centered culture throughout the organization. Professional Responsibilities Maintain accurate and timely documentation within the Family Registry database. Ensure adherence to HIPAA requirements, confidentiality standards, and organizational policies. Cultivate relationships with hospital transplant social workers and referral partners. Represent Transplant House at community meetings and partner events as assigned. Contribute to program development, resource creation, and continuous improvement efforts. Attend staff meetings and organizational meetings as required. Comply with all applicable local, state, and federal regulations. Perform other duties as assigned. Work Environment This is a fully on-site position based at the Transplant House campus in University Circle. The Social Worker works independently while collaborating closely with staff and volunteers. The role offers considerable autonomy in managing priorities, developing programming, and determining the best approaches to supporting guests. Responsibilities include regular interaction with transplant patients and caregivers, attendance at weekly guest breakfasts and dinners, and occasional local travel to hospitals and community partner organizations. Qualifications Required Master of Social Work (MSW) Minimum of 3-5 years of professional social work experience, preferably in a healthcare, medical, or transplant-related setting Strong interpersonal, communication, and relationship-building skills Excellent organizational and documentation abilities Ability to maintain confidentiality and exercise sound professional judgment Proficiency with Microsoft Office and virtual communication platforms Ability to work independently while managing multiple priorities Commitment to compassionate, guest-centered service Preferred Familiarity with organ transplantation and transplant-related care Experience working with patients and families facing serious illness Experience facilitating support groups and volunteer engagement Physical Requirements The employee must be able to: Walk up three flights of stairs Sit, stand, and walk for extended periods Operate office equipment and small tools Lift and carry up to 30 pounds Work Location Transplant House of Cleveland 2029 E. 115th Street Cleveland, OH 44106 Equal Opportunity Employer Transplant House of Cleveland is an Equal Opportunity Employer.
