Social Worker

Position Summary:

The Social Worker is responsible for supporting the emotional, social, and psychological well-being of residents within the nursing home. This role involves conducting psychosocial assessments, coordinating care plans, advocating for residents' rights, facilitating family communication, and ensuring smooth transitions during admission and discharge processes.

Key Responsibilities:

Conduct initial and ongoing psychosocial assessments for each resident. Develop individualized care plans as part of the interdisciplinary team and update them routinely. Provide counseling and crisis intervention to residents and families as needed. Assist residents and families with the admission, adjustment, and discharge/transfer processes. Collaborate with nursing, therapy, activities, dietary, and medical teams to address resident needs holistically. Advocate for resident rights and help resolve grievances or concerns promptly and empathetically. Coordinate community resources and referrals, including mental health, legal aid, guardianship, or housing support when appropriate. Maintain accurate documentation in compliance with federal, state, and facility policies. Support the facility's compliance with regulations related to social services, care planning, and resident satisfaction.

Qualifications:

Education:

Bachelor's Degree in Social Work (BSW) required; Master's Degree in Social Work (MSW) preferred.

Licensure:

State licensure or certification preferred

Experience:

Minimum 1-2 years in a long-term care or healthcare setting preferred.
