Job Description

QUALIFICATIONS
Must have a minimum of an Associate's degree (Bachelor's preferred) and meet requirements for Help Me Grow Service Coordination Credential. Must possess (at time of hire) and maintain throughout the course of employment a current state motor vehicle operator's license and have acceptable driving abstract. Must submit to background checks as a condition of initial and continued employment, as specified by policy.
WORKING HOURS
8:00am to 4:00pm, 260 days per year
DESCRIPTION
Serves as the single point of program contact for coordinating Early Intervention eligibility and services. Assist parents in gaining access to and coordinating the provision of early intervention services that their children and family need. Makes home visits, informs caregivers of their rights and obtains informed consent before moving forward with their child's referral. Makes referrals to providers for needed services and schedules appointments. Coordinates evaluations and assessments. Facilitates and participates in the development, implementation, review, and scheduling of meetings for IFSPs. Coordinates, facilitates, and monitors the delivery of early intervention services to ensure compliance with the IFSP. Coordinates funding sources for needed early intervention services. Facilitates the development of a transition plan from early intervention to special education preschool, if appropriate, or other services. Displays teamwork and cooperation. Works effectively with, or around, individuals with disabilities and their family members or guardians. Models appropriate social and ethical behavior. Attends trainings as required to remain up-to-date on issues relative to job duties and responsibilities. Works assigned schedule, exhibits regular and punctual attendance, and works overtime as approved to meet workload demands.
