It is our collective goal to evolve, providing quality services to meet the ever-changing needs of the community. Cadence Care Network has an opening for an Early Intervention Service Coordinator.
Responsibilities Include:
Provide support and consultation services to referred families of infants and toddlers with disabilities or developmental delays in Franklin County, OH. Take primary responsibility for the development and implementation of the Individual Family Service Plan(IFSP). Coordinate all necessary services/treatments, referrals, and assessments. Work in collaboration with community providers and provide referrals for additional services the family may need. Attend weekly team meetings with community providers to ensure continuity of care. Attend trainings with community partners and stakeholders as it relates to Early Intervention.
Qualifications:
Bachelor's degree in human service related field. Experience working with children ages 03. Valid Driver License, Auto Insurance and the provision of a vehicle for work purposes.
