Description:
- The Medical Social Worker is responsible for the provision of consultation and service to homebound individuals in the need of assistance with the solution of complex social and emotional problems, which impede management of medical regimen.
- Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Values in behaviors, practices, and decisions.
- Initially performs a comprehensive assessment of the patient and care-giver needs, and formulates appropriate plan of care.
- Establishes specific goals and regularly evaluates progress toward these goals.
- Demonstrates ability to identify and is sensitive to the comprehensive needs of patients and families.
- Develops with the patient and caregivers, a person-centered plan of care that will promote a holistic approach to the optimal medical, emotional, safety and well-being of patient and care-giver.
- Continually evaluates patient's needs and modifies care plans appropriately in collaboration with patient and care-giver.
- Develops a plan of care determined by the assessment process with goals established for the appropriate outcomes in collaboration with the physician
