Job Description
Why do this job? If you want to help folks in the most vulnerable population from Delaware County live healthy and meaningful lives, then the Delaware County Guardianship Services Board is the place for you. In accordance with R.C. § 2111.52, the Delaware County Guardianship Services Board was created in 2022 to administer a guardianship program serving incompetent individuals from Delaware County through appointments by the Probate Judge. The Delaware County Guardianship Services Board is appointed guardian and administers guardianship services through the utilization of Social Workers. The Social Workers will meet the needs of our clients by always acting in our folks' best interest, regardless of the situation.

A bachelor's degree in social work, counseling, education, health and human services, behavioral health services, or related field of study is required. Individuals with an LSW or LPC licensure will be given preference; 1-3 years of social work, case management, service coordination, or other related work experience is required; Proficiency in using a smartphone and telecommunication applications, Microsoft Office, video conference scheduling on virtual platforms, setting up and operating a personal computer, and adapting to changes with technology; Ability to effectively and professionally communicate verbally, and in writing, with diverse audiences; Patience, objectivity, maturity, effectiveness under pressure, initiative, and adaptability; Good time management skills, ability to prioritize work assignments, and act independently; Professional appearance and demeanor; Demonstrated dependability, reliability, and excellent attendance record; Ability to work in non-traditional environments; A valid driver's license, reliable vehicle, acceptable driving record as stipulated by the county's insurance requirements, and personal vehicle insurance; and, Willingness to submit to a criminal background check.PREFERRED QUALIFICATIONS
Working knowledge of local or regional treatment and service providers; Experience working with the Delaware-Morrow Mental Health & Recovery Services Board, the Delaware County Board of Developmental Disabilities, Delaware County Jobs and Family Services, the Central Ohio Area Agency on Aging, and/or SourcePoint; Experience working with the Probate Court of Delaware County in the area of guardianship; Core coursework in areas related to behavioral health, substance abuse, developmental disabilities, and/or elder care; Experience in direct clinical care, courts, or related fields; Experience working with developmentally disabled individuals, individuals with substance abuse disorders, individuals with behavioral health disorders, or individuals with decreased mental capacity; and, Experience in working with and collaborating with other agencies in the provision of care (e.g., physicians, housekeeping, travel, personal hygiene, law enforcement, mental health agencies, adult day care, recreation, attorneys, dentists, etc.). Maintains a case load and develops care plans to identify services and any other supports that would benefit the client; Documents and maintains accurate and up-to-date records using a case management system; Collaborates with community agencies, which may include but is not limited to: outpatient behavioral health providers, DD providers, nursing facilities, and hospitals to assist with care coordination; Conducts investigatory/wellness visits with clients monthly and as necessary based upon the needs of the client; Develops a one-to-one supportive relationship with clients through collaboration with various county agencies, community partners, and other service providers to ensure client progress and success; Prepares Court reports in a timely manner and works closely with representatives of the Delaware County Probate Court to remain in compliance with Probate Court rules; Assists the Director in preparation for Court appearances and appears/testifies in Court when required; Meets regularly with the Director to review the caseload and consults on day-to-day and major life decisions regarding clients; Effectively communicates with families and other social supports involved with clients; Makes consent decisions as a designee of the Board in accordance with Board policy; Promptly respond and/or meet with clients, courts, providers, medical facilities, county agencies, and other community partners outside of normal business hours, including evenings and weekends, based upon the needs of the client, and is part of a regular rotation of workers that are on-call after normal business hours; Participates in training, consultation, and activities that enhance the Board, its mission, and individual professional development; Comply with all supervision, licensure, and training requirements designed for the position; and, Performs other duties as assigned by the Director.PHYSICAL EFFORT AND WORK ENVIRONMENT
