Job Description
Duties:
Knowledge, Skills, Abilities Under the supervision of the FCFC Director, provides direct services and supports to Allen County FCFC Intersystems families. Essential Duties and Responsibilities include the following. Other related duties may be assigned: 50% Review Intersystems referrals received from community sources, follow up with referring individual, complete intake process with family/guardian, schedule team/family meetings, and assist with team communication. Complete CANS assessment to identify youth/family needs and strengths (renew every 90 days); create Plan of Care based on family voice and team input. Check in with family and team lead for updates to the Plan of Care. Work directly with Allen County FCFC partner agencies and community resources to build a team for each youth, and connect families with resources. 30% Coordinate Intersystems Committee meetings, create fund requests, update case list report (from OASCIS), and communicate with committee. Maintain case records in OASCIS, enter Plan of Care, and update goals and notes, as required. Enter service coordination and support costs into OASCIS for monthly submission. Work with FCFC Director and Intersystems Committee to review/update the Service Coordination Mechanism (SCM) annually. 20% Attend Council and task force meetings, as assigned and/or assist with Council cross-systems development trainings, and other duties assigned.

Experience:
Minimum of an Associate's degree in a related field or related experience; bachelor's degree preferred. One to three years of experience working with children and families preferred.

Create and maintain a family-centered plan designed with measurable goals. D. Direct collaboration among family-serving agencies; serve as key contact in scheduling family team meetings and communicating with families. E. Maintain case records using State-provided software (OASCIS). Organize and prioritize work assignments, multi task with accurate focus and refocus in a fast-paced environment. F. Ability to collect documentation and prepare fund request paperwork as part of the Intersystems Committee approval process. G. Knowledge of the Allen County social service system and community resources. H. Thorough knowledge, adherence and aptitude to follow policies, procedures, and practice related to safety, confidentiality, and federal/state/county laws and regulations.

Personal Work Relationships:
The Service Coordinator has daily and frequent contact with co-workers, public officials, employees from other public and private sector organizations, and the public. The purpose of these contacts is to guide and direct, review and assure progress of work assigned, coordinate services and handle questions about Council, community/regional/state services, and internal and external service coordination concerns.
