Job Description
Minimum pay range starts at $20.75 per hour. Pay dependent on experience. Starting at 35 hrs./week and increasing to 40 hrs./week as caseload builds. Early Intervention serves children ages 0 to 3 in need of services to address developmental delays or concerns. The Service Coordinator provides information and support to guide families through the eligibility process, through ongoing Early Intervention services and through transition services at age three. The Role of the Service Coordinator includes:
- Engaging families, arranging and conducting home visits, coordinating the evaluation and assessment process for eligibility, ensuring families understand their Rights as Parents within the Early Intervention system and connecting families to specialized Early Intervention Services and other community resources as needed
- Facilitating and participating in the development and monitoring of each child's Individualized Family Service Plan (IFSP)
- Participating in Early Intervention Primary Coaching Team Meetings to ensure families are receiving seamless and supportive Early Intervention Services
- Maintaining documents, filing, completing data entry on each eligible child as required
- Ensuring that services meet all required timelines including the initial 45 days, timely receipt of services and transition activities
- Obtaining initial credential as an Early Intervention Service Coordinator in Ohio within 30 days of hire Only candidates who meet the following minimum qualifications should apply:
- Bachelor's degree from an accredited college or university in a specific field (see below); or in any field with at least two years of verified full-time (or equivalent part-time) supervised, paid experience in case management and/or working with children birth through age five with disabilities, developmental delays, or diagnosed physical or mental conditions that have a high probability of resulting in a developmental delay and their families.
- Knowledge of typical child development, atypical child development and community resources
- Working knowledge of Microsoft Office applications and use of Internet
- Valid Ohio Driver's License and appropriate insurance coverage
- Awareness of Early Intervention services in Ohio Specific fields include: audiology, child and family studies, child development, child life, counseling, education, hearing and speech sciences, human development, human ecology, human social services, medicine, nursing, nutrition science, occupational therapy, ophthalmology, optometry, physician's assistant, physical therapy, psychology, rehabilitation, social work or speech language pathology.
