Job Summary:
Join the St Luke North Canton team as a Licensed Social Worker (MSW / LSW) in North Canton, Ohio, and be part of a mission-driven organization revolutionizing care delivery. As a key member of our healthcare team, you will have the opportunity to provide high-quality social work services to patients and their families, while collaborating with healthcare professionals to deliver comprehensive care.
Responsibilities:
Conduct social work assessments, develop, and implement individualized plans to meet patients' psychosocial needs Provide counseling, education, and support to patients and families facing chronic illness, mental health issues, or other challenges Collaborate with healthcare professionals to develop and implement treatment plans, promoting holistic patient care Identify and address social determinants of health that impact patient outcomes, such as housing, employment, and nutrition Participate in care coordination and case management activities to ensure seamless transitions and optimal patient outcomes
Requirements:
Master of Social Work (MSW) degree from an accredited institution Current licensure as a Licensed Social Worker (LSW) in the State of Ohio Knowledge of social work theories, practice models, and evidence-based interventions Excellent communication, interpersonal, and problem-solving skills Ability to work effectively in a fast-paced, dynamic environment
