Job Description

Primary Function/General Purpose of Position Provides home visitation services to expectant moms and families with young children that cultivate and strengthen nurturing parent-child relationships, promote healthy childhood growth and development, and enhance family functioning by reducing risk and building protective factors. Essential Job Functions Provides culturally sensitive home visitation services for families that promote positive parent-child interaction, child development skills, and health and safety practices using a curriculum and other educational materials in accordance with best practice model standards. Provides information, referrals, and linkages to available community resources. Links families to a medical provider to assure optimal health and development (i.e., timely immunizations, well-child care, etc.). Administers developmental assessment tools and screenings to monitor progress of child health and development and parent-child interaction skills in accordance with timelines. Works with families to develop individualized family service plans and to meet stated goals. Maintains accurate and complete electronic, case file, and program records in accordance with timelines. Facilitates relationships and coordinates services with program collaboration partners and other community agencies. Participates in supervision on a regular basis and participates in required trainings. Licensing/Certification For Early Head Start grant program- Child Development Associate (CDA) Credential from Council for Professional Recognition and the Family Advocate Credential (required within 18 months of hire unless they have a degree or coursework that meets the requirements in the Education section) For Mother Infant Early Childhood Visitation and Help Me Grow grant programs- must obtain Home Visitor Credential within 90 days of employment and maintain credential throughout employment. Education Associate Degree in Social Work, Early Childhood Education/Development or Nursing (required) Bachelor Degree in Social Work, Early Childhood Education/Development or Nursing (preferred) A combination of post-secondary education and experience will be considered in lieu of degree Work Experience 2 years of experience working with the community with an emphasis on home visiting (preferred)
