  • $62/hr W2 Responsibilities of Social Services Conduct patient assessments and develop individualized care plans.
Coordinate care and collaborate with healthcare teams, patients, and caregivers. Facilitate discharge planning and connect patients with community resources and support services. Provide patient and caregiver education regarding care plans and available benefits. Maintain accurate documentation of assessments, care coordination activities, and patient progress. Requirements for Social Services Minimum 1 year of skilled Home Health experience required. Active LMSW licensure in both Oregon (OR) and Washington (WA) required. Current BCLS certification required. OASIS experience required. First-time travelers welcome. Must have reliable personal transportation for patient visits.
COVID-19
primary vaccination series and one booster required. Experience with EPIC and Home Health documentation systems preferred. Ability to travel within the assigned service area as needed.
