Social Worker Responsibilities:
Assist Director of Social Services, as assigned, in completing a social history within four (4) days of resident's admission to the facility. Assist Director of Social Services, as assigned, in completing the psycho-social assessment portion of MDS within fourteen (14) days of resident's admission to facility. Assists Director of Social Services in writing progress notes on all residents every ninety (90) days and also on transfer to hospital, room changes within the facility, level of care changes, on discharge and post discharge.
Social Worker Requirements:
Must possess a Bachelor's degree from a school of Social Work accredited by Council on Social Work education with a minimum of 1 year employment in long-term care facility. Must demonstrate a working knowledge of the purpose and function of Social Work in long-term care facilities, as well as the psycho-social aspects of aging, and theory of human behavior and relations. Must demonstrate a working knowledge of social welfare systems, community resources and services, and health care insurances available to the elderly and their families. Must demonstrate an appropriate understanding of relationships between and among residents, families and Nursing Center personnel.
