Job Description
Seeking a Full time Social Services Coordinator to provide the delivery of direct and ongoing Social Services to residents and their families. Critically examines and evaluates the medically related social needs of each resident at admission and throughout their stay and in discharge planning. Social Service plans of care are developed, implemented, and documented for each resident. Must have a bachelor's degree in social work or a bachelor's degree in a human services field including but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology. 1 year experience preferred in a health care setting working directly with individuals.
