A well-established skilled nursing facility in Eastern Pennsylvania is seeking a compassionate and experienced Social Worker to join our interdisciplinary care team. This is a confidential search for a key role supporting residents, families, and staff in a collaborative clinical environment. The Social Worker plays a vital role in resident advocacy, discharge planning, psychosocial support, and care coordination while ensuring compliance with state and federal regulations.

Key Responsibilities:

Provide psychosocial assessments and ongoing support to residents and families Assist with discharge planning and transitions of care Collaborate with nursing, therapy, and administrative teams Support residents with adjustment, behavioral, and emotional needs Maintain accurate documentation and ensure regulatory compliance Participate in care plan meetings and family conferences

Qualifications:

Bachelor's or Master's degree in Social Work (BSW or MSW required) Prior experience in a skilled nursing or healthcare setting preferred Strong communication, organization, and advocacy skills Knowledge of long-term care regulations a plus
