Care Coordinator - School Based Services Program Children's Crisis Treatment Center - 1.8 Philadelphia, PA Job Details 11 hours ago Qualifications Working with children Working with families Working with individuals with mental health conditions Full Job Description The SBS Care Coordinator's primary responsibility is to manage the health and proactive relationship with children, families, and school communities. The SBS Care Coordinator will work to maintain a census/caseload that meets the standards as set in the program budget to provide care coordination support to children and families in the program.
Summary of Essential Functions:
Assess the service needs based on a Physician/Psychologist evaluation by completing the IBHS intake Identify, link, and coordinate services including preparation and distribution of IBHS packets Ensure children are CBH eligible to participate in IBHS Facilitate access to services Monitor the effectiveness of services. Reassess service needs Maintain appropriate documentation of all care coordination/case management services provided in the electronic health record, Carelogic Meet with the client and responsible parent within 1 week of the initial contact and maintain follow-up meetings weekly Establish and maintain coordination with County MH/MR, Children and Youth, schools, day care, recreational, legal or other child serving systems to ensure continuity of care and increase number type or resources available to the child/family Work collaboratively with the clinical treatment team (BC, MT, BHT) to ensure services are coordinated Support with entering outcomes data (CANS, Arizona) into Carelogic Assume other duties assigned by Clinical Supervisor/Lead Clinician and/or Team Leader Requirements Bachelor's degree in social work, Social Science, Bachelor of Science in Nursing, or Bachelor of Arts Degree from an accredited educational institution. Minimum of one (1) year case management, counseling, or direct care mental health experience with children and families.
