Our Dementia Care Navigators serve as advocates, educators, and steady guides for patients living with dementia and the caregivers who walk beside them every day. As a GUIDE Dementia Care Navigator, you'll: Develop individualized, person-centered care plans Serve as the primary point of contact for patients and caregivers Provide regular check-ins (in-person and by phone) Connect families with community resources, transportation, meal programs, and support groups Offer education on dementia progression and behavioral management Help coordinate respite services to reduce caregiver burnout Navigate complex healthcare systems and coordinate across providers Ensure timely and compliant documentation in alignment with

requirements This is hands-on, relationship-driven work. You won't be sitting behind a desk all day — you'll be in facilities, on the phone, and in the community, building trust and making real impact.
