Social Worker

We are currently seeking a compassionate, experienced, and dynamic Social Worker to lead our social work department and support the emotional and psychosocial well-being of our residents.

Key Responsibilities:

Oversee all aspects of the Social Services department, ensuring regulatory compliance and high standards of care. Provide support to residents and families during admissions, discharges, and transitions of care. Develop and implement individualized care plans addressing the psychosocial needs of each resident. Serve as a liaison between residents, families, staff, and external agencies. Participate in interdisciplinary care planning and quality assurance programs.

Qualifications:

Bachelor's or Master's Degree in Social Work (BSW or MSW) required. Minimum of 3-5 years of experience in long-term care, healthcare, or related field. Prior leadership or supervisory experience is strongly preferred. Strong understanding of federal, state, and local regulations related to social services in a healthcare setting.
