Social Worker:

Bachelor's Degree in Social Work or Human Relations field (sociology, counseling, psychology, special education, rehabilitation, etc.) At least one year of supervised social work experience in a skilled health care facility preferred but not required Strong documentation skills, communication skills with residents, families, staff, and community partners and ability to work independently and as part of a team Key Responsibilities of a

Social Worker:

Admissions :

Support new residents and families during the admission process; complete social history, care plans, and discharge planning documentation.

Care Planning :

Participate in interdisciplinary care conferences and update resident care plans and involve families in care planning, offer support, and provide guidance on community services.

Discharge Planning :

Coordinate follow-up services and provide resources to residents and families.

Documentation :
