Job Description
We are seeking a dedicated and compassionate BRIDGE Case Manager/Outreach Worker to join our dynamic team. This role is responsible for family-centered case management and proactive community outreach services to pregnant women, postpartum women, and pregnant women with dependent children who are engaged in, or in need of, substance use disorder treatment services. This hybrid role supports maternal health, promotes child safety and stability, addresses barriers to recovery, and builds strong referral pathways into services across the region. The staff member uses trauma-informed, culturally responsive, and family-focused approaches to help clients achieve recovery, parenting goals, and long-term self-sufficiency while also increasing community awareness of available services and strengthening partnerships with referral sources. Responsibilities Conduct comprehensive assessments to identify clients' medical, social, legal, childcare, housing, and recovery-related needs. Develop individualized service plans that address client goals related to recovery, prenatal/postpartum health, parenting, and family stability. Provide ongoing case management sessions to monitor client progress and adjust goals as needed. Assist clients in obtaining prenatal care, primary care, behavioral health services, WIC, SNAP, childcare services, legal aid, and other needed community resources. Conduct targeted outreach to pregnant and parenting women with SUD and related risk factors in the community, including outreach at clinics, hospitals, social service agencies, probation offices, and other community settings. Develop and distribute outreach materials (flyers, brochures, presentations) to reduce stigma and increase awareness of services. Build and maintain relationships with referral sources (e.g., OB/GYN offices, FQHCs, hospitals, WIC offices, DCFS, courts, community agencies) to promote program services and streamline referrals. Participate in community events, health fairs, and coalition meetings to share information about the program and increase visibility and access for PWWDC. Qualifications Bachelor's degree in social work, human services, psychology, or related field required. Experience working with pregnant women, families, or individuals with substance use or mental health disorders preferred. Experience in outreach, community-based work, or engagement with high-risk or underserved populations preferred. Knowledge of community resources for maternal health, childcare, housing, and family support. Ability to pass required background checks and drug screening. Valid driver's license and car insurance required. Reliable transportation and ability to drive during day time or evening hours required.
