Sentara Health Plans is hiring a Full Time Care Coordinator for the Tidewater Road,VA area!
Hours:
Full Time (40 hrs/wk)
Shift:
Day (8am-5pm) This position requires travel for in person face-to-face assessments in the Tidewater Roads area Days without assessments scheduled employee can work remotely. Applicants must be located in the Tidewater area The Care Coordinator performs a variety of casework duties and provides case management services to patients, families, and designated caregivers. Must develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses Education Bachelors Degree (Required) Bachelors Degree must be in Health & Human Services or one of the following related
