Job Description

Pediatric Case Management Social Worker Location:
Santa Barbara, CA (Hybrid/Remote)
Department:
Medical Management Overview We are seeking a compassionate and experienced Pediatric Case Management Social Worker to support children and individuals transitioning to adult care services with complex healthcare needs. This role focuses on coordinating care, connecting families to community resources, and supporting transitions into adulthood. Key Responsibilities Conduct psychosocial assessments and reassessments of pediatric members Develop and manage individualized care plans Coordinate care across providers, agencies, and community services Support transition planning (ages 16-21) for pediatric members Connect members to: Housing resources Government programs Behavioral health and community services Provide crisis intervention resources and support Act as a liaison between healthcare teams and external programs (e.g., CCS) Maintain an active caseload and ensure follow-up on member needs Collaborate with physicians, nurses, and community partners
Qualifications Required:
Master's degree in Social Work, Psychology, or related field 3+ years of clinical experience (LCSW can substitute for 2 years) Strong care coordination/case management experience Excellent communication and documentation skills
Preferred:
Experience in managed care / Medi-Cal / Medicaid Pediatric or special needs population experience Knowledge of community and behavioral health resources Bilingual (Spanish) CCM certification Additional Requirements Ability to travel locally for meetings Valid driver's license Comfortable working with families in sensitive situations Strong computer skills (Excel, Word, Outlook) If you are interested please apply here or email your resume to ggarcher @actalentservices.com! Job Type & Location This is a Contract position based out of Santa Barbara, CA. Pay and Benefits The pay range for this position is $50.00 - $55.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment.
