Job Description

Case Manager / Caseworker (Independent Living Program - ILP) Position Summary The Case Manager/Caseworker is responsible for coordinating, implementing, and monitoring services for youth in the Independent Living Program (ILP). This position focuses on assessing needs, developing service plans, and ensuring youth progress toward independence. This role also has potential to support adoption-related services as needed. Qualifications Bachelor's degree from an accredited college or university in a human behavioral science or related field, with a minimum of 25% of coursework in human behavioral sciences Job Responsibilities Working directly with youth, families, and service providers to develop, implement, and review individualized service plans Assessing youth needs, strengths, and readiness for independent living Monitoring and supporting youth progress toward independence goals Coordinating services including housing, education, employment, and community resources Conducting regular face-to-face contact with youth and documenting progress per MDHHS requirements Collaborating with Life Skills Coaches, support staff, and external providers to ensure continuity of care Ensuring compliance with MDHHS, licensing, and agency requirements Maintaining accurate and timely documentation, reports, and case records Advocating for youth needs and appropriate services Supporting transition planning for youth aging out of care Skills & Competencies Strong assessment and case management skills Excellent communication and interpersonal skills Ability to work independently and collaboratively Strong organizational and documentation abilities Ability to engage youth and families effectively Knowledge of adolescent development and independent living needs
Work Environment & Schedule Pay:
$22-25/hr This position could be part-time or full-time by taking on additional responsibilities. Some hours will include evenings, weekends, and holidays.
Work Location:
Hybrid remote in Fennville, Michigan Salary is negotiable, depending on experience.
Pay:
$22.00 - $25.00 per hour
