Case Manager - Social Worker

• Important:

  This position is located in Rockingham County, NC (Reidsville, Eden, Madison/Mayodan/Stokesdale)
• NOT Rockingham, NC
• Social Worker/Case Manager needed for multi-disciplinary In-Home assessment/care management team serving the elderly and disabled adults in Rockingham/ Guilford/ Caswell Counties. A Human Services degree and/or BSW or MSW in Social Work is preferred. Your typical day will be busy and varied
• you may be doing home visits at client homes, monitoring health, safety, and wellbeing, mentoring a family through a difficult care scenario, or back at your base completing supporting documentation. The Case Manager also completes case management tasks such as linking, assessing, informing, and coordinating services to prevent nursing facility placement. Your schedule is flexible
• our core office hours are M-Thurs and on Fridays
• but you set your own daily schedule based on your priorities and compliance deadlines.

Work from home option is available, but some hours will need to be spent at the office each week. This role will require some experience working with basic office computer software and technology (word, email, telephone system, fax machine etc.) We offer excellent benefits with generous paid holidays and time off.
